Abstract
Introduction: The prognostic impact of TP53, STK11, CDKN2A and KEAP1 co-mutations in EGFR- or KRAS G12C-mutated non-small cell lung cancer (NSCLC) remains unclear. Methods: This retrospective study included 190 patients (162 with stage IV disease). Molecular and clinical data were analysed for overall survival (OS), progression-free survival and response. Results: In EGFR-mutated NSCLC, TP53 co-mutation was associated with numerically shorter OS under tyrosine kinase inhibitors (22 vs 27 months). In KRAS G12C-mutated NSCLC, median OS was 18 months with co-mutations versus 12 months without. Among co-mutated patients, OS was 20 months with immune checkpoint inhibitors plus chemotherapy versus 9 months with monotherapy. None of these differences was statistically significant. Conclusion: Co-mutations showed heterogeneous prognostic associations. Validation in larger cohorts is needed.

Schlussfolgerung: Komutationen zeigten heterogene prognostische Assoziationen. Größere Kohorten sind zur Validierung erforderlich.
