Abstract
HINTERGRUND: Die Ösophagusatresie (EA) ist eine angeborene Fehlbildung der Speiseröhre, die häufig mit tracheobronchialen Anomalien assoziiert ist. Ziel der Studie war die Untersuchung des diagnostischen Stellenwerts und der Sicherheit einer standardmäßig präoperativ durchgeführten Laryngotracheobronchoskopie (LTB). METHODIK: In einer monozentrischen retrospektiven Observationsstudie wurden alle Neugeborenen mit EA untersucht, die von April 2005 bis Oktober 2021 am Klinikum Oldenburg behandelt wurden. ERGEBNISSE: Bei 54 von 62 Patienten wurde eine präoperative LTB durchgeführt. Zusätzliche tracheobronchiale Fehlbildungen wurden bei 7 Patienten detektiert. Peri-interventionelle Komplikationen traten nicht auf Schlussfolgerung: Die präoperative LTB ist ein sicheres Verfahren mit hohem diagnostischem Nutzen. Sie ermöglicht die genaue Fistellokalisation, erkennt zusätzliche Atemwegsanomalien und unterstützt die operative Planung. Die Ergebnisse sprechen für eine routinemäßige Durchführung bei Neugeborenen mit EA.

Evaluation of standardized preoperative laryngotracheobronchoscopy in children with congenital esophageal atresia

BACKGROUND: Preoperative laryngotracheobronchoscopy (LTB) enables localization of tracheoesophageal fistulas (TEFs) and detection of airway anomalies in neonates with esophageal atresia (EA). This study evaluated its diagnostic value and safety. Methods: In this retrospective single-center study, 62 neonates with EA treated at Klinikum Oldenburg between 2005 and 2021 were analyzed. RESULTS: Vogt type IIIb was the predominant subtype (90.3%). LTB detected airway anomalies in 11.3% of patients and localized the TEF in all but one patient, predominantly dorsomedially (94.4%). No LTB-related complications occurred. CONCLUSION: Routine preoperative LTB is a safe and valuable diagnostic tool that facilitates detection of airway anomalies and surgical planning. Its routine use should be considered in all neonates with EA.
