Abstract

Die fachärztliche Versorgung der rheumatoiden Arthritis (RA) ist seit Jahren im Blickpunkt der Wissenschaft. Über Patienten ohne fachärztliche Anbindung ist hingegen wenig bekannt. Ziel dieser Dissertation war es, die Routineversorgung aller Patienten mit RA abzubilden. Als Datengrundlage dienten Abrechnungsdaten der BARMER GEK, in denen Versicherte mit RA identifiziert wurden. Aus ihnen wurde eine Zufallsstichprobe gezogen und befragt. Durch das Linkage der Abrechnungs- und Befragungsdaten standen administrative, klinische und patientenberichtete Daten zur Verfügung. Fachärztlich betreute Patienten erhielten deutlich häufiger Antirheumatika. Mit steigendem Alter nahm allerdings der Anteil fachärztlich und medikamentös versorgter Patienten ab. Die Ergebnisse unterstützen die Forderung nach einer Erhöhung der Rheumatologenzahl, die allein die Herausforderungen aber nicht lösen wird. Es müssen auch Konzepte zur Verbesserung der Versorgung alter und immobiler Patienten entwickelt werden.

Analysis of the care of patients with rheumatoid arthritis from the adolescence to high age: results from claims data linked to a questionaire survey

The specialized care of rheumatoid arthritis (RA) is in focus of medical science since years. However, data on care of patients without specialized care are rare. The aim of this dissertation was to study the routine care of all patients with RA. Claims data of the German statutory health insurance fund BARMER GEK were available for the identification of patients with RA. A random sample was drawn from all insurants with RA and questioned. A linkage of claims data and survey data allowed extensive analyses of administrative, clinical and patient-reported information. Patients in specialized care got more frequently anti-rheumatic medication than in unspecialized care. Independently of the physician rheumatologic care and anti-rheumatic medication decreased with increasing age. The results of this study support the call for more rheumatologists. However, an increase would not change the elementary challenges. Concepts on care of older and immoveable patients had to be created as well.
