Abstract

D-negative Mütter mit D-Antikörpern weisen bei einer Schwangerschaft mit einem D-positiven Kind ein großes Gefährdungspotential für Komplikationen vor und während der Geburt auf. Eine erste Risikoabschätzung kann über die Bestimmung der RHD-Zygotie des Kindsvaters erfolgen. Erstmals ist ein solcher Nachweis durch die quantitative Amplifikation des RHD-Gens und Ko-Amplifikation des RHCE-Gens möglich. Eine direkte, risikofreie Diagnostik einer feto-maternalen D-Inkompatibilität ist mit Hilfe der TaqMan-Technologie durch den Nachweis von Spuren fetaler DNA im maternalen Plasma durchführbar. Die Untersuchung auf genetische Risikofaktoren für Koronarthrombosen mit einer neu entwickelten PCR-ELISA-Methode konnte keinen direkten Zusammenhang zwischen den untersuchten Parametern (HPA-1, Gerinnungsfaktor V, Prothrombin, ACE) und dem Auftreten eines Myokardinfarktes aufweisen. Allerdings konnte gezeigt werden, daß es zu einem signifikant höherem Herzinfarktrisiko bei einer Kombination von Diabetes und dem HPA-1b Typ kommt und die Frequenz der G1691A-Mutation des Gerinnungsfaktors V mit der Zahl stenosierter Koronargefäße korreliert.

The TaqMan technology was utilized to assess pregnancies at risk of RhD incompatibility. Quantification of the RHD-gene together with the RHCE genes as internal reference differentiated between homozygous and heterzygous RHD individuals. The TaqMan PCR combined with a new extraction procedure provided the sensitivity to detect DNA from a few D-positive fetal cells in the plasma of D-negative mothers. ELISA detection of PCR products generated with labeled primers was used to investigate genetic risk factors in samples from 851 patients with coronary artery disease. The history of a myocardial infarct (MI) was not asscociated with the HPA-1b, factor V(G1691A), prothrombin (G20210A), ACE (deletion/insertion) polymorphisms. HPA-1b was however associated with an increased risk of MI in the subpopulation of diabetic man younger than 60 years. The number of stenosed coronary arteries in the angiogram was correlated with the frequency of the factor V mutation.
