Abstract

Das postoperative Delir (POD) ist sowohl mit einer erhöhten Mortalität als auch einem verlängerten Intensivaufenthalt vergesellschaftet. Hierbei konnte gezeigt werden, dass eine zentrale Komponente der Pathogenese ein ACh-Mangel im basalen Vorderhirn darstellt. Die Steuerung des ACh-Spiegels erfolgt neben der Sekretion über die Cholinesterasen (ChE). Alteration innerhalb ihrer Aktivität können dabei mit dem Delir vergesellschaftet sein sowie einen möglichen Therapieansatz bieten. Diese Arbeit untersuchte die perioperative ChE-Aktivität bei 51 herzchirurgischen Patient:innen sowie einem möglichen Zusammenhang zum POD. Hierbei zeigte sich in der Studienpopulation eine hoch signifikante Abnahme der ChE-Aktivität perioperativ (p<0,001). Patient:innen mit einen POD wiesen dabei präoperativ hoch signifikant niedrigere ChE-Aktivitäten auf (p<0,001). Eine angeschlossene ROC-Analyse ergab hierfür eine AUC von 0,815 (95% Konfidenzintervall = 0,697-0,932; p>0,001).

The blood cholinesterase activity as a predictor for the development of postoperative delirium in patients undergoing cardiac surgery

Postoperative delirium (POD) is known to be associated with both, longer ICU-stay and longtime mortality. A central component in the development of delirium is a disbalance in neurotransmitter, especially a cerebral ACh decline. The elimination of ACh is based on cholinesterases (ChE). Therefore, the current research on delirium focuses on alteration in ChE-activity. It may also be a potential target for pharmacotherapeutic agents in delirium therapy. The main aim of this study was to detect perioperative alterations in ChE activity. Furthermore, to identify possible association between these alterations and the POD. Within the study population of 51 patients undergoing cardiac surgery, a highly significant decrease in ChE activity was found perioperatively (p<0.001). By comparing patients with and without POD there was a significant difference in preoperative ChE activity detected (p<0,001). Based on these data, a ROC analysis showed an AUC of 0,815 (p>0.001).
