Abstract

Die Bestimmung der Pfannenposition nach Hüft-TEP anhand von Röntgenaufnahmen ist essentiell hinsichtlich der Zertifizierung zum EndoProthetikZentrum und stellt ein Qualitätskriterium dar. Ermittelt wurde der Zusammenhang zwischen Messwerten der Inklination und Anteversion intraoperativ durch einen Navigationsassistenten und konventionellen Messungen anhand postoperativer, röntgenologischer Aufnahmen. Ziel war hierbei der Nachweis einer möglichst hohen Korrelation beider Messmethoden. Bei 642 Patienten wurde der Inklinations- und Anteversionswinkel anhand von postoperativen Aufnahmen bestimmt und mit Werten des Navigationsassistenten korreliert. Sowohl die Messreihen der Inklination (0,335) und Anteversion (0,198) zeigten eine schwache Korrelation und einen statistisch signifikanten Unterschied (p≤0,05). Ein Einfluss des BMI auf Messunterschiede beider Methoden konnte für die Inklination und die Anteversion nicht nachgewiesen werden. Insgesamt ist bei angenommener hoher Messgenauigkeit des Navigationsassistenten die Bestimmung der Pfannenposition anhand röntgenologischer Aufnahmen kritisch zu diskutieren.

Retrospective investigation on the correlation of radiological results and navigation data of the cup position after total hip arthroplasty

Determining the acetabular cup position based on total hip arthroplasty using X-rays is essential with regard to certification for the EndoProthetikZentrum and represents a quality criterion. The connection between measured values of inclination and anteversion determined intraoperatively by means of a navigation assistant and conventional measurements using postoperative X-ray images was detected. The aim was to demonstrate the highest possible correlation between the two measurement methods. In 642 patients, the inclination and anteversion angle was determined on the basis of postoperative images and correlated with the values of the navigation assistant. Both, the measurement series of inclination (0.335) and anteversion (0.198), showed a weak correlation and a statistically significant difference (p≤0.05). An influence of the BMI on measurement differences between the two methods could not be demonstrated for inclination and anteversion. All in all, assuming that the navigation assistant assumes a high level of measurement accuracy, the determination of the cup position on the basis of radiological images should be discussed critically.
